Hi!
Any ideas how to implement the following. 

On my forum I have some kind of HELP section. 
So only in this section while creating new thread, ti bring up some template to help to describe the problem in details. 

* please describe condition 1
* please describe condition 2 and so on
* please do not forget to insert photo to your post.

Sorry english is not my first language but I hope you getting the idea.
