What is the URL of your forum? Also, paste here credentials to the test-account.

I'm expecting you are running this on local server for testing. follow the steps to debug the issue.
1. Goto http://yourforumurl/forum-teamchat.php and check if the page is working properly or not. If it is working properly, goto your templates group and find forumteamchat_iframe template and modify <iframe> line to the working page.
2. If the page is not working. Do you see any errors logged? if yes, let us know the error. If not create a .htaccess file in your forum's root directory and put following content in the file.
RewriteEngine On
php_value display_errors 1
refresh the forum-teamchat.php page and show us the error you are getting.
