I had previously installed the "Cool Ajax Registration" mod but it is now gone. (not just deactivated)
Is there something I have to change back to fix the captcha image?
I would revert the templates that "Cool Ajax Registration" changed for installation. I don't remember what those are off of the top of my head.
member_register and member_register_password, but I already did that.
Make sure this is in the template for member_register:

Has it always worked or never? This is very strange...
It worked a long time ago, but then none of us registered for a long time. I bet it has something to do with the cool ajax register though.
Have you tried re-activating the plugin then seeing if it works? If it works you know it's the plugin.
Well the plugin has been gone for a long time, but i reactivated and deactivated it earlier today. It didn't work, but it didn't work the first time either, due to php limitations.
Can you post your exact template here? Do you have access to any error logs that you could check?
