Can someone explain this for me please?

Admin CP > Forum Settings > Show Thread Options > Read Threads in Database (Days)

What does "Read Threads in Database" mean? Should I disable it?

If I understand the system correctly, it normally marks all threads as read, except for those with posts made since your last visit. With "Read Threads in Database", it will track your last visit to each individual thread, up to X number of days, after which it will assume that thread is read.
