I've browsed through the MOD section, and done a few searches but have failed to turn up any results what I'm looking for. On the website www.mybbfans.com the contact page (misc.php?action=contact) and it has a specific contact form which looks like this.

I was wondering if someone could tell me which version of the Contact Form this is, and where I can get it?

have you tried to ask them (mybbfns)?

They never released that to the public even though they said they would. I eventually made one of my own. It's in the mod database and it's very similar. You can edit it to be exactly like that one if you want.
