Hi everyone i need help to make this feature possible with mybb and my website

my forum is located to forum/
my website is at the rootdir.

i what this form are included into my website at www.mysite.com/ask.php

this form will post on this x forum id exemple if forum id = 10 then is in this forum
Thread subject as automaticly set to "ticket: Name, lastname, and date"

in the message display will be like
exemple...

Name: Jean
Lastname: Terrian

Answer of question (Dropbox): Question 1

Checkbox (but unchecked)
question 1 : no
question 2 : no
....

and the message of the scroll text area

... this is my idea and i dont know where to start i really need this new feature. thanks in advance.

BTW, the in this case the location of the file doesn't matter because you can put it in the forum directory and then redirect the metioned url to the real file with .htaccess.

that's an exemple for the link... , i just want a post thread like but i can include to my website as like a form for contact email but with few options then its post on the forum with specifict title etc, but i tryed to do it but i guess im too bad with this new forum before in the past i was used phpbb.. but seriously btw mybb and phpbb i prefer mybb aswell this forum is a lot of options and expandable. i just love it -_- but i have to learn myself too soon to make my own stuff. thanks bro.
