I have this setting set to 'no' for the forum. However, sometimes I have to approve the attachment and sometimes I don't. No sure why. Am I missing a setting somewhere else?

It may be set differently for the forums.

Check each forum to make sure Moderate New Attachments is set to "No."

It seem to affect one forum only.
