I have problem when use Mod: Subforums Columns 2.0

My forum use: MyBB 1.2.9

http://mods.mybboard.net/view/subforums-columns-2.0

I can't see all my Sub-Forums in Index page.
It likes this:

How I can config for display all Sub-Forums.

Like this :

* One more question:
How I config my Index board display like this:

Admin CP --> Board Settings --> Change --> Forum Home Options --> Subforums to show on Index listing

Admin CP --> Forum Management --> Manage Forums --> *Choose forum to edit or when creating a new forum ensure that 'Act As Forum?' is set to 'No'
