I think there should be atleast a option for members when they edit their post it says:

Edited By: tgo42596 at 6/11/08 Because: Reason goes here

Or something like that.

I think this is better suited as a Plugin. An edit reason is pretty annoying.

Personally, I'd prefer the moderator/user specify the reason in the post that was edited, or a subsequent post explaining the reason for the edit.
I see little benefit of having fancy extra fields for something like this.
