MySQL error: 1062
Duplicate entry '1' for key 1
Query: INSERT INTO mybb_modoptions (uid, cpstyle, notes, permsset, caneditann, canmodposts, canban) VALUES ('1', 'Axiom', '', '1', 'yes', 'yes', 'yes')

Not a MyBB problem, but a problem with your plugin.
