How can I have set it so every user has a specified default avatar (rather than just blank) if they haven't chosen one?
Easy. Set a default in the users table of mysql (phpmyadmin) the avatar column for an image like blank.gif. Upload blank.gif to your images/uploads/ folder and bingo.
