Every time, now, one of my long time users, gets his user group changed from:
Moderator --> Administrator
without his knowledge.

But it's not even the same color as Administrator role (his name looks red, but adminstrators are bright red with bold and underline).

And when this happens, he can't login.
I have to manually reset his role to Moderator again for him to log in.
Then it goes back to administrator again without anyone doing anything.

Strangest bug ever.

I tried rebuilding caches and permissions, still no luck.

Check your group promotions.
In ACP-->Users & Groups->Says something like Group Promotions.

Ok, yes the guy had a lot of reputation, so he was being promoted to administrator (which was suppose to happen) but it still doesn't explain why he couldn't log in anymore and why his name wasn't highlighted as an admin, but instead highlighted as a normal no-span user.

Check the display group of the user. It is probably still set as Moderator.

I can't explain the login problem though. You could take a look at his password hash in the database, and then take a look at it after he gets automatically promoted. The password hash should not change during the promotion, which means the password you enter should be the same (if you have an unmodified version of MyBB)
