i know the password, so how should i fill out the install info, i use mysql

You should ask your host about the hostname if you are unsure, you can leave the other two as they are unless you want to change them, and know what changing them will do. It's fine to leave them as they are.
I have hostmonster, maybe someone else knows it?
It might say in the Host control panel, and if not, email them... sometimes it's just localhost, sometimes it's something more than that.
