Looks like you didn't read the readme file. I wonder what they're for..
Simply upload the mp3 folder provided in the download, then add and configure the mycode with the codes provided in the download package. You're done. Then you can either see if it works on the sandbox (on the mycode page) or just try it on a new post. Oh and you asked for a step by step guide right?
Here's one:
Download the BBCode package, then right click readme.htm and select open from the menu. Tcharam, you're done.

Check if you deleted the folder or the mycode in the ACP.
Other than that it SHOULD be working. Also it depends if the mp3 file is broken or not.

Probably because it is no longer compatible with the latest releases of MyBB.
