If I try to upload an avatar over an existing one, it doesn't show my new avatar. Rather, it shows the previous one resized to the size of the one uploaded.

Example:
[1]My current avatar is 120x120.
[2]I try to upload a new avatar: 100x100.
[3]My current avatar is now [1] 100x100.

Have you tried refreshing the page a few times? I have that here on MyBB when I try my avatar - my browser's cache doesn't seem to like it!

The only way it's every worked for me was to supply a URL to the image instead

Yes, because when you upload an avatar, it still uses the same file name; your browser needs to recache it.

Try a hard refrresh.
