Could be one of three things:
- You've upgraded from 1.2 to 1.4 with 1.2 plugins active, which you shouldn't do. Hopefully you made a full file and database backup of the 1.2 forum, I'd revert to that, deactivate plugins, and upgrade again.
- You're trying to use a 1.2 plugin on a 1.4 forum, which won't work.
- Check all your database details are right in ./inc/config.php, specifically the database name and table prefix.

No, i was using 1.4.5, All of a sudden i get this.

Sorry, updated my post. The double table prefix error you have is generally caused by one of those 3 things.

fixed, i just removed all the plugins, weird though, since i had 1.4 before and it was working a bit after i upgraded
