I received the following error when I've submitted the "add warning" form to a user.

Mybb version 1.4.6
MySQL 5.0.77

As a moderator in some forum i receive this error every time when warning user. The operation is completed and the user warn is saved but i must every time to comeback to the home page. The Admin of the site don't succeeded to resolve this problem and i want to help him. I suppose the reason is some script added or changed but if someone have an idea about the direction to search the solution, please help.
