This should be something simple, but unfortunately I can't figure out how to create a moderated user group (i.e. their posts require approval).

It can't currently be done on a user or usergroup basis, only on a forum basis, although, based on the level of people that request it, it may work it's way into the next major release.

OK thanks. I can understand why a lot of people would request it, as it would be handy.

One other thing which is related - between 1.2 and 1.4 what was changed exactly with the approve/unapprove moderator tool? I know what it does now, but it didn't seem to do anything before?

You could change it slightly so it puts a usergroup other than 1 into moderation :p
