Just a tiny issue. In the file usercp.php find:

$leavelink = "<div style=\"text-align: center;\"><a href=\"usercp.php?action=usergroups&leavegroup=".$usergroup['gid']."&amp;my_post_key={$mybb->post_code}\">".$lang->usergroup_leave."</a></div>";

& should be &
