It says on the download page to install
Easy Install first. You need then only upload the paypal plugin to the inc/plugins directory and click activate.
where do i put the ei file?
In the /inc/plugins directory

Sorry for the late reply. If you need help with the settings then you can install Advanced Plugins which gives you access to a readme file in the plugin.
The link IMPAQ provided will setup the PP donate button. But you also have to setup Instant Payment Notification in PP. Either look at PP's help section or install Advanced Plugins to get more information.
