I want to change my new thread settings and thread settings so you have the option to make it a review, if you do you can add custom options (up to 4) a name and a subname, an image (250x250 px) and a listing of either songs or actors and an optional preview link, and the writer can also add their rating out of 5 stars which will display under the text.
Then in the outcome I would like it to look like this:

How hard would this be - if even possible, so far me and another friend who is good at PHP spent a day trying to figure it out but could not.
