It would be a good idea if the standard MyCodes like Bold and Italics would be in the MyCodes list in the admin cp. That way they can be customised.
Also, an additional option to add an icon for the editor would be great.

You can't but imagine the url bbcode. You can add attributes like rel="nofollow" and target="_blank".
It would also allow me to remove those hardcoded things in the img tag as well as add anything missing (it's been a while since I last tried so I can't remember which) to make it xhtml 1.1 strict compliant.
