Being allowed to upload your own post icon to a thread would do that.
Yes, i have seen this at many boards myself.
This one has the image as part of the topic tittle it seams.

I would also like it, Here is what i think would be best.
It would auto use the FIRST image in the post and auto size the image to a desired size set in the acp.
Then it would be displayed in the threads page as above. Just my 2 cents.. but i would LOVE to see that done
