ah, I totally for got about that, because I tested it on my forum at localhost, sorry about that.

Yes, you have to change the http://localhost to your website's link and the http://localhost/convert to your forum's link.

P/S: I'll change the attachment, so anybody that download later won't have to ask again lol, sorry about that.
