Is there a Mod where it automatically ban the user for a period of time if they get an "x" amount of warnings?

You can setup a warning level to ban them when they reach a certain warning % which is effectively the same thing. ACP > Configuration > Warning System (side menu).

Thanks, didn't know that. Also, is there a way to "reverse" warnings?

You can revoke them by viewing the warn report (click the warning level of a user, choose the warning, click View) and revoke it by entering a reason in the box at the bottom and submitting.

When the user hits that warning level do they get a permanent ban or what ?

Yes, they will be permanently banned. But you can edit their bans either from modcp or from acp.
