I want to edit this page:
http://www.myforum.com/warnings.php?action=warn&uid=2
- I want to remove "Custom Reason"
- I want to make "Send User a private message notifying them of this warning" mandatory.

You can remove the "Custom Reason" via
ACP -> Configuration -> Settings -> Warning System Settings -> Allow Custom Warning Types?
Set to no.

If anyone knows how to do the other, I would like to know as well Smile

If I have understood go to acp->templates & style->templates->your template->warning system templates->warnings_warn_pm->find: 
{$send_pm_checked}
Replace with: 
checked="checked"
