From the Wizard I set the database name to 'foo' but in the function select_db I get the error

Unknown database 'foo'

I was assuming the myBB would set things up in mysql. Do I need to do something first with mysql?

You have to create the database first, MyBB doesn't create it for you or 'set anything up'.
