listen these information should be provided to you by your hosting company, or at least when making the database and its users, you should remember them
simply
server remains localhost
database : is the name you have called you new data base ( sometimes you need to add the prefix)
User: is the user you have given the privilage to him to use the db
pass: is thr pass you have used for the mentioned user.
table prefix: whatever you want, or what have been provided by your hosting company.
