Im trying to find a form addon/plugin where you can create your own form.
Basically what i am looking for is a sort of form which could be used as an application to join our group where we can add around 8 questions(ish) which they must answer to apply and of course their details which we will require for them to join.
Is there such an addon which could be added to my forum?

That will work great as I use it on ShockingHost.com . You can do almost everything with it

exactly what i am looking for but i have a little problem with it - i have requested on the designers forum but havent heard a response.
the form needs to be for people who are not registered for the forum, i have set the groups to "guests etc" so i would have expected anyone could use it but when someone who isnt logged in clicks the link, it asks them to log in which isnt what i want.
is there a way around it, i have a recruitment user which i thought maybe i could use the user id to be able to post the form onto the forum but dont know how to get around the non-registered people can use the form.
