If you are still having problems, can you save the source code of the web page (Menu: View->Source) and attach it as a text file please?

I've just thought. If there has been a problem with some part of the install, the form button will not be created. Make sure there were no errors.
descwzaqz is correct, but it should be displaying. Do you know of anything that would cause a fatal error? Any other type of error would still continue to parse the page correctly, so im not too sure whats wrong except that the image is not read able or does not exist
