Make sure you stored it in the correct place
I wouldn't be worrying about it too much if it's just one smilie. But try re-uploading it, and making sure the path to the smilies is correct.
Not sure what we can do with this level of information. URL and test account so we can see it?? Chances are the image is just missing...
