Do you have your forum URL in the "Default profile website URL", like "http://mysitehere.com"?

If using the previous solution is not working for some reason, then use this code instead and you will see your default avatar:

<img src="{$mybb->user['avatar']}" height="55" width="55" >

Yes i saw the reviews, but sometime people believe that a plugin is not working when it has only been wrongly configurated. Mostly because of insufficient installation instructions...

Have you tried the solutions i have suggested?
