Bare in mind if you have a large forum, this will add a lot of extra queries, and slow down your board.

I tried it on the default theme but it did not work and here are two different mod .

I tested on localhost . Can you give a demo image ?
I activated and post new thread have images and attachment image but nothing appears when the mouse hover .

Have you tried it with a regular registered member account rather than an admin account?

Oh, this mod is only tooltip text,no image in the tooltip. Sorry you did not read carefully the guidance of this mod.
The Mouse over Thread Preview 2.0 mod is better but it was too old, not compatible with mybb1.4.x

Zinga's plugin (mentioned by AJS) does not add a lot of extra queries.

I haven't been able to check the code out, but I'll take your word for it
