What page are we talking about? Is this a throttle of some kind?

Yes. Basically tasks are triggered by code in the footer, run whenever anybody visits a page on the forum. The mass mail task is set to run every 15 minutes by default so every 15 minutes it sends out however many you set here, 50 by default I believe. Next time it's run, sends the next 50.
