Did you create your MySql database for your Forum? This is where you will be able to find out your username/password and hostname! Usually with your web hosting you have a Control Panel/Dashboard and you will be able to create your database there.

You will need to set this up before you can continue!
Unfortunately we don't know what they are any more than you do. As explained above, these details should be made clear in the place you create your database, but if not, you'll need to ask your host.
A related question to databases. How many database rows does a standard MyBB 1.6 install have before plugins are added? Is it 74?
Think you mean tables rather than rows; it has 70.

So the other 4 in my case would have been added in by plugins?
