If i make a Moderator the group leader of the Moderator group, how is he/she supposed to manage the users of the group? The only group management I have seen is in the Admin CP...

User CP > Group Memberships.

how does a member join a group if it is not open to freely join but is managed?

I have a group that has a leader, so they should be able to add/accept/reject people... but were does a member go to apply for the group?

If it's not free to join they can't join it... you can't add any arbitrary person to the group, you can just approve/reject/remove people who are in it or want to join it. People apply to join the group in the exact same place, they'll have a list of groups they're allowed to join.
