Is there a working captcha mod so that when users sign up they have to get the image letters correct, or answer a simple question?

Captcha is a built in feature, you just have to enable it. There is a plugin for additional questions during signup.

You'll have a hard time to stop bots from invading your forum. Bot coders can break Captcha in 2-3 minutes. You can't stop them. Just for your information, try to install Captcha II for better security against spam.
