Is there a way to create a custom form on the post page that asks for certain information, then displays it on the post after a user submits it?

Possible, yes. You will need to find someone to make a plugin for that.

It depends on what you are trying to set up - look at the example setups on the XThreads site ( http://mybbhacks.zingaburga.com/forumdisplay.php?fid=31 )and then use the templates provided.
