Dear friends,
Is there any plugin to allow users to fill a form (that can contain textboxes, checkboxes, radiobuttons , etc.) . I want to ONLY FORUM USERS can fill that form. (NOT to force users)

how big of a form and what do you want to do with the data once submitted?
