Here's the scenario.

I have given super-mod permissions to the default moderators group.
They were shown in the Forum Team pages under Moderators tab as desired.

Then I assign someone who does not belong to the moderators group to moderate a single forum.
Now in the Forum Team page under the moderator tab, only that single person which was assigned a forum shows up. The members of the moderators group don't.

I hope I was clear.

Because that assigned user is a moderator, so it should be shown on forum team.

I'm not sure if that answers the OPs question - what about the super moderators who used to show up and now don't?

Maybe connected to this?

Yes it should be shown and it does.
But the super-moderators who are actually in the moderators group do not show up anymore.

Still looking for a solution.

I can't reproduce your problem, however when I assign a user as a moderator of a forum, they don't show up on the Forum Team page at all...

No, they do. :O
They are shown in the Forum team in their own group color but under the moderators tab in Forum team page.
