I have it enabled in ACP > config > forum display options

But there is no bar that says "Todays Birthdays: 0"

Even though there are no birthdays I still want it to be there. Does this mean that this only works/shows when there is a birthday? How can I make it to show all the time even if there aren't any?

Yes, it only shows if there are actually some birthdays, you'd need to change the code or request a plugin to have it show all the time.
