Ok I have the Page Manager plugin which lets ma make pages, but I need a form where users can type in their name and email address and whatever and click submit and it will be recorded and such. How/where can I get a script that does this so I can add it to my forum using page manager?
How do you specify what the email address to send the emails to is?

It will send emails to Forum Admin's email.
Oh okay, and the Admin's email is the one specified in the Admin's profile account or the email in ACP > Settings > General Configuration > Admin Email ?
If its the one in the profile of the admin, what if there is > 1 admin?
