Forum URL (No trailing slash): would this be www.mydomain.com/forums.php? and whats a trailing slash?





Website Details

Website URL: www.mydomain.com (right???)





Cookie settings (?)



Cookie Domain: I have no idea what to put here.





Cookie Path: I have no idea what to put here





Contact Details (Shown in footer)



Contact Email: i know this

Just leave the cookie settings as they are, they're pre-filled for a reason, you don't need to change them.
