EDIT: It would be more helpful if you would have posted the whole code as is, by just removing the parts you have colored in the image ;-)
Windows (Notepad) expects lines to be ended with both \r (CR=carriage return) and \n (LF=new line feed) characters while Unix-based systems only use \n to end lines. This causes the file to appear as one line in Windows Notepad. Use a more modern text editor to edit PHP files, like Notepad++.
