Header and footer, that way people don't hate you. You can do inside a post, or in between, but users don't like that.
I usually do header, footer, and in between the thread and quick reply box.
my analytics show sidebars are most effective. Its also the opinion of Google in their suggestions to webmasters.
Anyways, it depends on your site.
