Your whole headerinclude is a mess. You've got HTML that belongs in the body and everything in there...

Are you only having problems with the inline moderation? The collapses are working fine (which would normally also be broken).

This problem seems to be happening randomly with some of my themes where the popup login won't work.. I just noticed it for the first time today not working (all the themes I've made never had a problem before until today for me).
