It's not exactly hard. The Nudges plugin in use on runegear.net has a few extra features though (such as user tagging and post liking I believe).

It notifies people if someone likes your post, tags you in a post with a custom tagging system, replies specifically to your post or replies to your thread. All this is built in and integrated.
