Thats right, if it doesn't have a .com/.net/.org/et cetera, then the cookie domain should be left blank.

Upload the .php file to inc/plugins folder of your MyBB. Then go to Admin CP --> Plugin Manager and activate the plugin. Make sure the setting for the plugin in the Board Settings is set to "On" (I didn't notice where the setting is inserted or if it's already on by default).

it is off by default and it is found (both of them) in the general configurrations section of biard settings
