DVZ not appearing?

The plugin DVZ Shoutbox http://community.mybb.com/mods.php?action=view&pid=63 I have followed all steps as it said but the shoutbox is not appearing on my forum.
First of all, I copied the content from STYLES.CSS in .zip to global.css
Then I put {$dvz_shoutbox} in Index Templates> Index of my theme.
Then I activated the plugin. But the Chatbox is not appearing on the homepage of my forum. I am using 18MySkins theme.
