General Display Issued

I have a few general display issues.

The top bar doesn't look right.

And the tables look like they don't have headers.

See pic. Also, when posting, there is no button to add attachments.

Have you tried reverting the templates back to default and see if that helps ?

That was the first thing I tried. It does this with the basic template and any other template I install.
